Agreement between Prism Massage & Wellness and Customer
Welcome to Prism Massage & Wellness (the "website"). This website is provided solely for customers to schedule appointments for our massage therapy services and to provide information and details about our services. The terms "we", "us", "our" and "Prism Massage & Wellness" refer to "www.prismwellnesssaz.com", “prismwellness.as.me”, and/or Prism Wellness, LLC and its agents. The term "you" and “your” refers to the customer (also known as the “client”) visiting our website, mobile site, and/or booking massage therapy appointments through us on this website, via phone, and/or through our agents. This website, related platforms, and our services are offered to you conditioned upon your acceptance without modification of all the terms, conditions, and notices set forth below (collectively, the "Agreement"). By accessing or using this website in any manner, contacting our agents, or conducting business with us, you agree to comply and be bound by the Agreement. If you do not accept all of these terms and conditions, please do not use this website, related platforms, or conduct business with us. We reserve the right at any time, at our sole discretion, to change or otherwise modify the Agreement without prior notice, and your continued access or use of this website, related platforms, and our business services signifies your acceptance of the updated or modified Agreement. Our Services & Upgrades
Prism Massage LLC (doing business as Prism Massage & Wellness) is a massage studio in Tucson, AZ that provides massage therapy services by licensed massage therapist(s).
Massage therapy is defined as the manual manipulation of the muscles and other soft tissues (such as lymph or fascia) of the human body by a trained and licensed massage therapist to relieve pain, lessen stress, promote relation, and improve physical functioning.
Massage therapy is not intended to diagnose, treat, cure, or prevent any medical condition or disease. It is not a substitute for professional medical advice, diagnosis, or treatment. If you have a medical condition, illness, or injury, you should consult a qualified healthcare professional before receiving massage therapy. Certain conditions may contraindicate massage, and it is your responsibility to inform your therapist of any health issues or concerns.
A massage therapy appointment (also known as the “session”) involves the licensed massage therapist providing massage therapy to you (known as the “client”) in a treatment room within the massage studio. Prior to the session, the massage therapist has an “intake” with the client to discuss the client’s pertinent health information and their goals and preferences for the massage session. The massage therapist leaves the treatment room to allow the client to undress and then lay onto the massage table under the provided sheets and blankets. Then, the massage therapist returns to the treatment room to start the massage therapy session. The massage therapist will maintain professional draping throughout the entire session. Draping in massage therapy is the technique of covering a client with a large sheet or blanket to maintain the client’s privacy and comfort during a session while exposing only the specific area being worked on at any given time. This method ensures that clients remain safe, warm, and unexposed, regardless of their level of undress, and prioritizes both physical modesty and emotional security. Once the session concludes, the massage therapist will leave the treatment room again to allow the client to redress. The client will then exit the room and pay for their service.
We offer the option to add upgrades to our massage services. Upgrades are additional enhancements that clients can add to their basic massage session to provide extra benefits and to elevate their experience. These upgrades can include using tools (such as hot stones, cupping therapy, or manual instruments), using a different massage medium (such as an oil, cream, or lotion that contains a beneficial ingredient like an aromatherapeutic essential oil or CBD), or extending your session time. Availability of upgrades is not guaranteed and must be confirmed at the time of booking or at the studio prior to service. Additional fees apply for all upgrades, and current rates will be disclosed before the start of the session.
We offer the option to purchase gift certificates and appointment packages for our services.
A gift certificate is a prepaid voucher purchased for the value of a specified massage service or dollar amount redeemable exclusively at Prism Massage & Wellness. Gift certificates have no cash value and cannot be redeemed for cash unless required by law. They are non-refundable and may not be used to purchase other gift certificates. Gift certificates do not expire and do not incur any fees for inactivity. Redemption is subject to availability of services, and the studio reserves the right to refuse service or reverse gift certificate redemption in cases of suspected fraud or dispute.
Appointment packages consist of a prepaid series of massage sessions sold at a discounted rate compared to individual session pricing. Packages are redeemable exclusively at Prism Massage & Wellness. Our package sessions have an expiration timeframe of 12 months from the date of purchase after which any unused sessions will expire. Packages cannot be redeemed for cash or refunded once purchased. Any discounts, specials, or promotions we offer cannot be combined with appointment packages.
Gift certificates and appointment packages purchased from Prism Massage & Wellness are transferrable at the sole discretion of the purchaser. This means the purchaser may choose to provide their gift certificate or package code or account information to family members, friends, or others for redemption of services. We are not responsible for verifying the identity or authorization of the person redeeming a transferred gift certificate or package. Customers assume full responsibility for any sharing or transfer of gift certificate or package access and any resulting use.
We reserve the right to modify, substitute, or discontinue any service, upgrade, gift certificate, or appointment package offering at our discretion. All services, upgrades, gift certificates, and appointment packages are subject to change in availability, pricing, and description without prior notice. Completed services are non-refundable per our refund policy.
Scheduling & Data
Appointments must be scheduled online at prismwellness.as.me, which is accessible through several links on our website. You are required to complete a client intake form that contains your phone number, email address, and personal health data. A card will be required to be put on file for your appointment to enforce our cancellation policy.
We record SOAP notes after each and every service to document the session. The acronym SOAP stands for Subjective, Objective, Assessment, and Plan, representing key sections of the note:
Subjective: Details what you, the client, report about your condition, symptoms, pain level, and concerns in your online client intake form and your verbal intake with your massage therapist. This section captures your reasons for seeking massage and your requests including focused massage on certain areas and/or your preferred pressure level.
Objective: Records measurable and observable data gathered by your massage therapist during the session, such as posture, range of motion, muscle tension, and other palpable findings.
Assessment: This section describes the massage therapy techniques and protocol your massage therapist uses during the session based on their evaluation of the subjective and objective data collected.
Plan: The course of action moving forward, including continuing treatment techniques used in the session and recommendations for future sessions such as session frequency or utilizing other massage therapy techniques to further progress.
Massage therapy SOAP notes are used so your massage therapist can maintain organized records of your sessions, including documentation of your progress, and to demonstrate professionalism and high quality care. You may request to access your SOAP notes at any time. Your information is confidential and required for proper assessment. Parents or guardians may book a massage therapy appointment for their child that is 5 years of age or older with the express permission and request of the child. The parent or guardian must provide their own contact information and their child’s health data when completing the client intake form. The parent or guardian must remain in the treatment room or wait in the waiting room for the entirety of the massage session.
If you are under 18, please do not use this site or provide any personal details through our booking forms, contact forms, or payment features without the express permission and supervision of your parent or guardian. If we learn that we have inadvertently collected information from a minor without verifiable parental consent, we will delete it as quickly as possible. Parents or guardians who believe their child has submitted unauthorized personal information through this website may contact us directly so we can remove the information.
Terms & Conditions for Service
By scheduling and receiving a massage therapy appointment, purchasing a gift certificate, or purchasing an appointment package from Prism Wellness LLC (DBA Prism Massage & Wellness) you agree to the following terms and conditions:
• You do not have any injuries or health conditions that prevent you from receiving massage therapy and you have notified your massage therapist of all known medical conditions, injuries, surgeries, and medications. You understand that your massage therapist is not liable for any damages that may occur due to information you've omitted.
• You understand that your massage therapist reserves the right to refuse, modify, or terminate service based on any contraindications you may have, including but not limited to acute injury, illness, severe medical conditions, contagious diseases, and poor hygiene.
• You understand that you are receiving a professional, therapeutic massage service and that any sexual, hostile, or unprofessional behavior is strictly prohibited. You understand that if you violate this policy at any point, your session will be terminated and you will be banned from scheduling future appointments with us and from contacting our agents.
• You understand that you must immediately inform your massage therapist if you experience any physical pain during your service so the pressure can be adjusted to your level of comfort.
• You understand that a time extension for your appointment is unavailable if you are late and that the session will end at the scheduled ending time.

Our Right to Refuse Service
We cannot provide services that are outside of our legally defined scope of practice as licensed massage therapists. We use your health data to properly assess and determine your condition to receive massage therapy, including possible contraindications. A contraindication is a physical, mental, or emotional condition that makes massage therapy unsafe or detrimental to the client's health, posing a risk of harm or worsening the condition. Contraindications can be absolute (massage should not be done at all), local (massage should not be applied to a certain area), or relative (massage may be done with caution or modifications, such as lighter pressure). Examples include acute injury, illness, infections, open wounds, severe medical conditions, contagious diseases, and poor hygiene.
We reserve the right to refuse, modify, or immediately terminate service based on any contraindications you may have. If service is refused due to a medical contraindication, we may request a signed release form from your doctor stating that you are medically cleared and deemed safe to receive massage therapy before continuing future sessions.
We also reserve the right to refuse or immediately terminate service for any behavior you exhibit that is deemed inappropriate, hostile, unprofessional, or as sexual misconduct. If you exhibit any form of behavior that is deemed as such during your service or while on our business premises, your behavior will be documented in your client profile and you will be banned from scheduling future appointments or conducting any form of business with Prism Wellness, LLC and its agents.
Cancellation Policy
By scheduling and receiving a massage therapy appointment, purchasing a gift certificate, or purchasing appointment package from Prism Wellness LLC (DBA Prism Massage & Wellness) you agree to our cancellation policy:
• If needed, please cancel or reschedule your appointment at least 12 hours in advance. You can do so by clicking the "Reschedule/Cancel Appointment" button in your appointment confirmation email.
• A cancellation with less than a 12 hour notice or a no-show will result in a charge equal to 50% of the service cost.
• To reschedule within the 12 hour window, please contact us. You will be sent an invoice to prepay the full cost of your appointment. Your new appointment time will be confirmed after your invoice has been paid.
Refund Policy
We do not offer refunds for completed services, purchased gift certificates, or purchased appointment packages. A refund may be issued if you prepaid for your service and request to cancel your appointment at least 12 hours before your appointment start time, per our cancellation policy.
If your service is prematurely terminated due to you exhibiting behavior that is deemed inappropriate, hostile, unprofessional, or as sexual misconduct, you will be charged the full fee for your service.
If your service is prematurely terminated due to a benign reason such as sudden illness or a family emergency from either party, you will only be charged for the elapsed time of your service.
Use of Website & Services
• You agree to use this website for lawful purposes only and not to engage in any conduct that is unlawful and/or could damage, disable, or impair the site or interfere with any other party’s use of the site.
• You are responsible for providing accurate information during account creation, appointment booking, or any communication with us.
• We reserve the right to refuse service or cancel appointments at our discretion.
Limitation of Liability
• Prism Wellness LLC (DBA Prism Massage & Wellness) and its owners, agents, employees, or massage therapists are not liable for any direct, indirect, incidental, consequential, or punitive damages or injuries resulting from your use of our websites, related platforms, or services.
• We do not guarantee uninterrupted or error-free access to the website and are not responsible for any technical issues beyond our control.
• The massage services provided are for relaxation, wellness, and therapeutic purposes only. They are not a substitute for professional medical diagnosis, treatment, or advice. Massage therapy is provided at your own risk.
Indemnification
You agree to indemnify, defend, and hold harmless Prism Wellness, LLC (DBA Prism Massage & Wellness), its owners, agents, employees, or massage therapists from any claims, damages, liabilities, costs, or expenses resulting from your use of our websites, related platforms, or services, your violation of this Agreement, or your violation of any rights of a third party.
Intellectual Property
All content on this website, including text, graphics, logos, images, and software, is the property of Prism Wellness, LLC or its licensors and protected by copyright and other intellectual property laws.
You may not reproduce, distribute, modify, or create derivative works without our express written permission.
Termination
We reserve the right to suspend or terminate your access to our website or services at any time, for any reason, without prior notice or liability.
Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to conflict of law principles.
